Job Summary:We are seeking a skilled Registered Nurse to join our team as a Wound Care Specialist. As a key member of our interdisciplinary health team, you will provide high-quality wound and ostomy care to clients and caregivers, utilizing best practices and current knowledge of wound care protocol.
Key Responsibilities:- Utilize the nursing process to provide high-quality wound and ostomy care for clients and caregivers.
- Collaborate with the interdisciplinary health team to provide cost-effective, high-quality wound care.
- Apply current knowledge and research of wound care protocol to practice.
- Create a plan of care for patients requiring wound care in the community, in consultation with the physician, CCAC Case Manager, ICCP coordinators, and Primary Nurse.
- Initiate and modify the plan of nursing care in consultation with the client and/or caregiver.
- Promote the independence of clients and caregivers, considering their needs, strengths, and weaknesses.
- Utilize available resources, facilitate clients/caregivers problem-solving, participation in decision-making, and access to appropriate resources.
- Participate in the evaluation and refinement of the Wound Care Protocol and overall program.
- Educate other members of the SJHC team on the protocol and usage of products, providing guidance to best treatment choices.
- Maintain regular communication with supervisor to include review of problems or unique issues and exchange feedback.
- Participate in bi-weekly nursing case rounds.
- Ability to work collaboratively with others; sharing expertise and drawing on the expertise of others.
- Actively promotes a healthy, supportive, and inclusive work environment.
- Proactively contributes to initiatives, supporting and encouraging positive change.

- Bachelor of Science Degree from an accredited university.
- Current and unrestricted Registered Nurse license.
- Member in good standing with Ontario College of Nurses.
- Proven ability in utilizing MS Office software, including Excel spreadsheets.
- Knowledge of community supports, Home Care community, Acute and LTC.
- Familiar with Procura software (preferred).
- Certification as a wound care nurse, or equivalent courses (recommended).
- Minimum of one (1) year recent experience in home care (preferred).
- Demonstrated excellence in direct client care.
- Excellent verbal, written communication, and interpersonal skills.
