Job Description:

We are seeking a highly skilled Field Case Manager to join our team at CB Canada. As a Field Case Manager, you will play a crucial role in ensuring the smooth delivery of patient care and support services.

Responsibilities:
  • Practice Management: You will work closely with prescribing physicians to customize program elements that meet their unique needs, ensuring adherence to College of Nurses standards of nursing practices.
  • Patient Support: You will welcome patients to the program, capture required information, and provide therapy guidelines to ensure seamless access to therapy.
  • Mentorship/Peer Support: You will assist in training new Field Case Managers, support peers through regular communication, and develop positive relationships with the Program Patient Support team.
  • Client Interface: You will participate in scheduled KOL physician meetings, develop strong working relationships with physicians and office staff, and proactively resolve client issues.
Requirements:
  • You must have a RN or RPN degree, registration with the Provincial College of Nurses, and case management experience.
  • You should have a minimum of three years of nursing experience in a clinical setting, availability to travel within an assigned territory, and a valid driver's license.
  • BCLS may be required depending on the therapeutic area of the program, and fluency in French and/or English may be necessary depending on the program and location.
