Job Title: Community Care Assistant - Vancouver Coastal Health

Job Summary:

As a Community Care Assistant with Vancouver Coastal Health, you will be an integral part of our team providing support to clients in their homes. We are seeking a caring and compassionate individual to join our team.

Key Responsibilities:

  1. Provide home support services to clients under the direction of the Supervisor/Coordinator/Manager or delegate.
  2. Assist clients with varied levels of care, observe, and report significant changes in the client's presentation.
  3. Collaborate with the healthcare team, clients, and their caregivers to maximize independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  4. Motivate, encourage, and involve clients in their care.
  5. Provide input and feedback in the care planning process to the healthcare team.

Qualifications:

Education & Experience:

  1. Graduation from a recognized Health Care Assistant Program.
  2. Current/Active Registration with BC Community Health Worker and Care Aide Registry.
  3. Experience interacting with persons with dementia, geriatric, and/or palliative population will be an asset.
  4. Must have Valid BC Driver's license and access to a vehicle, as local area travel may be required unless specified in the posting.
  5. Proficiency in Mandarin or Cantonese is required.
