As a key member of our Behavioral Health Services team, you will play a critical role in providing high-quality care to individuals with complex mental health and substance use challenges.

Key Responsibilities

  • Provide comprehensive screening services to newly admitted clients, assessing their mental health and substance use needs and prioritizing referrals accordingly.
  • Utilize evidence-based tools and assessments to identify and address mental health and substance use concerns, taking into account the unique needs and experiences of each individual.
  • Develop and implement individualized care plans, working collaboratively with interdisciplinary teams to ensure seamless transitions and continuity of care.
  • Collaborate with healthcare professionals, social workers, and other stakeholders to provide comprehensive support and services to clients.
  • Maintain accurate and detailed records, ensuring timely and effective communication with clients, families, and healthcare teams.

Requirements

  • Bachelor's degree in Social Work or a related field, with a minimum of two years of experience in a mental health and/or substance use care setting.
  • Strong knowledge of mental health and substance use disorders, as well as experience working with diverse populations, including Indigenous individuals and those with complex needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced, dynamic environment.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
