Job Summary

We are seeking a highly skilled Social Worker to join our team at Fraser Health. As a key member of our interdisciplinary team, you will provide clinical therapeutic services to patients and families in our assigned clinical specialty area.

Key Responsibilities
  • Conduct basic social work assessments of patients and families using a variety of assessment tools in accordance with professional standards, clinical policies and practices.
  • Provide consultation to patients, families and other healthcare professionals on issues related to high risk factors, recovery and current community practice relevant to the assigned clinical specialty area.
  • Develop and coordinate patient care plans, including arranging family meetings to assist in developing and coordinating patient care plans and community services for patients requiring facility placement and/or home support.
  • Communicate effectively with community health and social agencies, including government Ministries, Home Health personnel and family physicians, to collaborate in the assessment and management of individuals at risk.
  • Act as a resource to patients and their families by providing information and facilitating access to community resources.
Requirements
  • Bachelor's degree in Social Work from an approved school of Social Work, plus one year recent, related acute care clinical experience.
  • Current full registration with the BC College of Social Workers.
