About the Role

We are seeking a highly skilled and motivated Pharmacist (PT) to join our team at our Cochrane Safeway Pharmacy location. As a Pharmacist (PT) with Safeway, you will be responsible for providing exceptional patient-centered care and exceeding patients' expectations.

Key Responsibilities

  • Provide clinical services as appropriate to practicing top of scope in your province, including chronic health condition monitoring and management, medication reviews, health risk assessments, prescription renewals or modifications, minor ailments, or other prescribing services.
  • Initiate care plans, follow up, and complete associated documentation.
  • Collect relevant patient, drug, and medical condition information.
  • Identify and resolve actual and potential drug-related problems.
  • Monitor and optimize medication adherence.
  • Provide patient counseling that is tailored to the needs and concerns of the patient, including literacy, hearing loss, and privacy to ensure comprehensive understanding of the medication.
  • Understand all pharmacy offerings and provide each patient with a personalized interaction and intervention if appropriate.

Requirements

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy from a recognized institution.
  • Licensed Pharmacist in the province of responsibility.
  • Authorization to Administer Injections.
  • Excellent attention to detail, ability to multi-task, and work independently in a fast-paced environment.
  • Adhere to and implement all applicable company standards.
