We are seeking a highly skilled and compassionate Community Mental Health Nurse to join our team at Fraser Health. As a key member of our Psychosis Treatment Optimization Program, you will play a vital role in providing support and treatment optimization for individuals experiencing psychosis.
Key Responsibilities- Provide direct nursing care to complex psychotic clients with treatment-resistant psychosis, collaborating with psychiatrists and primary care physicians to ensure evidence-based planning and support.
- Assess and identify clients with treatment-resistant psychosis, collecting baseline demographics and performing physical, functional, and suicide risk assessments.
- Monitor blood tests and other laboratory results for adverse effects of medication, taking immediate action as per clinical protocols.
- Collaborate with psychiatrists, staff at PTOP clinics, and mental health centers to ensure seamless care coordination and communication.
- Provide client advocacy services, assisting clients and their families to access community resources and promoting person-centered care.
- Maintain accurate and up-to-date client records, documenting nursing observations, assessments, and treatment formulations.
- Participate in research and quality improvement activities, identifying client care issues and contributing to the development of evidence-informed practices.
- Graduation from an approved School of Nursing, plus two years of recent experience in a community or hospital psychiatric setting.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Valid B.C. Driver's license and access to a personal vehicle, with appropriate business insurance coverage.
- Knowledge of mental illness and substance use treatment, psychopharmacology, and legislation related to mental health practices.
- Excellent communication and interpersonal skills, with the ability to promote person-centered care and collaborate with interprofessional teams.
