About the Role

CarePartners is launching a new program in partnership with Ontario Health atHome Central East, seeking skilled Community Support Workers to join the High Intensity Supports at Home (HISH) program. As a valued member of the interdisciplinary care team, you will work with patients and their families to provide wrap-around care, supporting care while patients transition through the healthcare system.

Key Responsibilities

  • Assisting with daily living activities, including personal care, household tasks, respite care, medication assistance, and special functions.
  • Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety.
  • Providing cognitive, social, and/or emotional support to patients.
  • Preparing nutritious, varied, and economical meals and snacks, including special diets, as determined by the care plan.
  • Providing patient status reports and participating in patient/family care planning meetings.
