We are seeking a highly skilled Social Worker to join our team at Fraser Health. As a Social Worker, you will provide clinical therapeutic counseling services to patients and families in our assigned clinical specialty area.
Key Responsibilities- Conduct basic social work assessments of patients and families using a variety of assessment tools in accordance with professional standards, clinical policies and practices.
- Provide consultation to patients, families and other healthcare professionals on issues related to high risk factors, recovery and current community practice relevant to the assigned clinical specialty area.
- Develop and coordinate patient care plans and community services for patients requiring facility placement and/or home support.
- Communicate with community health and social agencies to assess and manage individuals at risk, make referrals to appropriate resources and exchange relevant information.
- Act as a resource to patients and their families by providing information on community resources and access to these resources.
- Participate in research activities, program planning and development relevant to the needs of the patient, families, hospital and community.
We are looking for a candidate with a Bachelor's degree in Social Work from an approved school of Social Work, plus one year recent, related acute care clinical experience. Current full registration with the BC College of Social Workers is also required.
The ideal candidate will have basic knowledge and understanding of emotional and mental health issues related to illness, injury, patient and family cultural dynamics. They will also have basic counseling skills, including crisis counseling, brief therapeutic interventions and loss and grief applicable to patients, families and other support networks.
