Responsibilities
As a Care Coordinator Specialist, you will be responsible for facilitating smooth transitions from hospital to community for individuals living with complex mental health, addictions, and health needs. You will provide service coordination, psycho-social assessments, intensive case management, therapeutic and/or supportive counseling, and referrals to various resources to improve outcomes for older adults, seniors, and their family/caregivers. You will work collaboratively with healthcare providers, community agencies, and other stakeholders to enable individuals who are vulnerable, at risk, and marginalized to live safely and independently in the community.

Key Responsibilities Include:
• Referrals and outreach: Identifying individuals who could benefit from social work support and transitional care coordination
• Service coordination: Working with hospital staff to identify and engage clients and support hospital staff in making referrals to the CCIS team, as appropriate
• Direct client support: Providing psychosocial and/or psychological and geriatric assessments, service/care co-ordination, system navigation, short-term and long-term planning, and follow-up support
• Therapeutic and supportive counseling: Utilizing a wide range of therapeutic approaches that best meet the clients' needs and circumstances
• Crisis intervention: Providing preliminary assessment of risk in crisis situations, consulting, and devising an intervention and action plan

Requirements
To be successful in this role, you will require a minimum of 3 years related experience in mental health, addictions, and/or social work. You must have demonstrated experience facilitating psycho-social groups and supporting individuals with significant mental health and/or addictions issues, cognitive impairment (e.g., dementia), and chronic health issues from a harm reduction, trauma-informed, and mental health recovery approach. You should also have experience in conflict mediation, client engagement, and community development.

Preferred Qualifications:
• Experience working with diverse populations, including those living with complex mental health and addiction issues, geriatric concerns, and other complex needs
• Knowledge of theories of addiction, evidence-based treatment models, and best practice approaches
• Ability to provide intensive and short-term case management, care coordination, and system navigation
• Strong de-escalation and crisis intervention skills
• Excellent oral and written communication skills and documentation skills
