Clinical Care Specialist

We are seeking a highly skilled Registered Practical Nurse (RPN) to join our team. The ideal candidate will possess excellent clinical skills, a strong commitment to delivering high-quality patient care, and a passion for working in a dynamic and supportive environment.

In this role, you will provide compassionate and individualized care to our clients in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each client.

• Provide comprehensive nursing care to clients in their homes, including medication management, wound care, and vital sign monitoring.
• Conduct thorough assessments of clients' physical and emotional needs, and develop and implement care plans accordingly.
• Collaborate with clients, families, and healthcare professionals to ensure seamless transitions of care.
• Document all interactions and interventions in accordance with Bayshore's documentation standards.
• Maintain accurate and up-to-date records of client information, including medical histories, treatment plans, and progress notes.

To be successful in this role, you must:

• Hold a current registration with the provincial licensing body.
• Have a valid driver's license and reliable vehicle.
• Demonstrate excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
• Show a strong commitment to delivering high-quality patient care and a willingness to go above and beyond to meet the needs of our clients.
