We are seeking a highly skilled and dedicated Registered Nurse to join our Primary Care team at Northern Health Authority. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the development of our team.
About the Role:- Provide direct clinical care and services to individuals, groups, and communities in primary care settings.
- Develop and implement care plans in collaboration with the interprofessional team.
- Conduct assessments, provide education, and support patients and families in achieving their health goals.
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.

- Three years of recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid BC Driver's License.
- Assessment and Intervention: Demonstrated ability to complete initial and ongoing client assessments and provide nursing care through appropriate interventions.
- Communication: Demonstrated ability to communicate effectively with clients, families, and healthcare teams.
- Critical Thinking: Demonstrated ability to integrate and evaluate pertinent data to problem-solve effectively.
- Person and Family Centered Practice: Ability to promote person-focused care that demonstrates care for and with people and their families.
- Teaching and Education: Knowledge of adult learning principles and facilitation skills to effectively educate patients and families.
