Job Summary

We are seeking a highly skilled and experienced Pharmacist to join our team at Fraser Health. As a Clinical Pharmacist, you will play a critical role in ensuring the safe and effective use of medications in our healthcare facilities.

Key Responsibilities
  • Interpret medication orders and prescriptions, and dispense medications using a computerized medication information system in accordance with legal, professional, and departmental policies, procedures, and standards.
  • Assess prescriptions for therapeutic suitability by referring to standardized references, interpreting laboratory data, and reviewing patient-specific demographics.
  • Review patient profiles for any patient allergies, potential drug interactions, therapeutic duplications, or contraindications, and withhold medications until safety concerns are resolved.
  • Communicate with physicians, patients, and other healthcare team members for prescription clarification and problem resolution, and document the outcome of interactions with prescribers.
  • Provide instructions to others on proper use, administration, storage, and disposal of medications, and communicate verbally and in writing with physicians, nurses, and other professionals.
  • Participate in obtaining medication history from select patients, verify allergy information, and assist with medication reconciliation processes.
  • Ensure that medications are efficiently and accurately dispensed and/or compounded and labelled, and that dispensing records are complete.
  • Provide medication counselling regarding drug therapy to patients and/or their families, as required, and provide allergy assessment and discharge counselling for appropriate patients.
  • Document patient interactions in the patient healthcare record in a standardized format, and review patient screening reports or targeted drug lists to identify potential drug-related problems based on available laboratory data.
  • Participate in the collection of data for medication use evaluation projects, and document required information.
  • Assist in maintaining adequate inventories, note shortages, requisition drugs and/or supplies, and monitor expiry dates, report procedural errors, and participate in quality improvement activities.
  • Participate in dispensary-based implementation of medication safety initiatives and quality control activities in accordance with established standards and procedures.
  • Maintain records for narcotic and controlled drugs, special access program drugs, and departmental statistics in accordance with legal, professional, and departmental standards, policies, and procedures.
  • Participate in the orientation of pharmacy students, residents, technicians, assistants, and new staff members by demonstrating dispensary-related functions, procedures, techniques, and department equipment, and provide verbal guidance and input regarding progress, as assigned.
  • Contribute to internal publications and participate in clinical drug trials by dispensing medications and completing associated documentation.
  • Review cases of reported drug adverse reactions and complete necessary documentation.
  • Participate in staff meetings, committees, and other programs as required, identify opportunities to improve processes and/or procedures to enable safe and efficient operations, and refer to supervisor and/or others.
Qualifications
  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
  • Knowledge of drug distribution processes in a hospital setting.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to establish and maintain effective working relationships with other healthcare practitioners, as well as patients, family members, and the public.
  • Ability to organize and prioritize work.
  • Ability to operate related equipment.
  • Physical ability to carry out the duties of the position.
