We are seeking a compassionate and skilled Registered Nurse to join our team in providing high-quality care to patients in their homes. As a Registered Nurse in our community care team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities
- Conduct thorough assessments of patients' needs and develop personalized care plans
- Deliver high-quality care to patients in their homes, including medication management, wound care, and education
- Collaborate with patients, families, and healthcare teams to ensure seamless care transitions
- Document patient progress and maintain accurate records

Requirements
- Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse
- Valid driver's license and access to a reliable vehicle
- Experience in community care or a related field (an asset)
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
