Nurse Care Coordinator

The successful candidate will be responsible for delivering high-quality patient care to our residents in a long-term care setting. This includes assessing resident needs, developing and implementing care plans, and collaborating with interdisciplinary teams to achieve optimal outcomes.
Key responsibilities will include:
* Assessing resident needs through consultation with medical professionals and internal staff
* Developing and implementing care plans that meet individual resident needs
* Collaborating with healthcare teams to achieve optimal resident outcomes
* Administering medications and treatments according to physician's orders and policy
We offer a competitive salary of $80,000 - $100,000 annually, depending on experience, along with a range of benefits including comprehensive health and dental coverage, RRSP contributions, and access to continuing education and training opportunities.
