Key Responsibilities
- Assess and coordinate care for clients post-solid organ transplantation, working collaboratively with interdisciplinary teams to ensure comprehensive treatment and follow-up.
- Develop and implement patient education programs, utilizing adult education principles and evidence-based practices to promote optimal health outcomes.
- Consult and confer with healthcare professionals to address complex health issues and develop effective care plans.
- Participate in multidisciplinary committees and contribute to the development of nursing practices and policies.
- Stay up-to-date with the latest research and trends in solid organ transplantation, applying this knowledge to improve patient care and outcomes.
Requirements
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years of recent experience in adult patient education or a related field, with a strong background in nursing theory and practice.
- Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Strong knowledge of adult education principles, evidence-based clinical practice, and research methods.
- Ability to work independently and as part of a team, with a focus on providing exceptional patient care and education.
