Community Rehabilitation Specialist

We are seeking a highly skilled Occupational Therapist to join our team at Fraser Health. As a key member of our mental health services team, you will provide specialized occupational therapy services to clients in their area of practice.
Key Responsibilities- Provide direct and indirect occupational therapy services to clients, including assessment, planning, and intervention.
- Collaborate with interdisciplinary teams to develop and implement treatment plans.
- Consult with and educate other professionals, agencies, and natural supports to promote client participation and recovery.
- Develop and maintain partnerships with community agencies to enhance client services.
- Conduct home assessments and community visits as required.
- Provide individual and group treatment to clients, including psycho-education, skill development, and other needs.
- Identify and develop treatment goals with clients and treatment teams.
- Provide input on program budgets and purchases as requested.
- Represent the program and service area on Fraser Health and local committees.
- Current full registration with the College of Occupational Therapists of British Columbia (COTBC).
- Valid B.C. Class V Driver's License and access to a personal vehicle for business purposes.
- Working knowledge of current psychiatric rehabilitation, psychosocial approaches, and recovery practices.
- Demonstrated ability to use activity analysis to assess occupational performance and modify the activity, environment, or supports to enable occupation.
- Ability to utilize outcome measurement tools.
- Demonstrated leadership and presentation skills.
- Ability to collaborate and work effectively in a community team setting, as well as independently.
- Ability to communicate effectively both verbally and in writing with clients, families, and other members of the health care team and community agencies.
