Case Manager, Indigenous Mental Health Specialist

We are seeking a highly skilled and compassionate Case Manager to join our team at Fraser Health. As a Case Manager, you will play a critical role in providing clinical services to Indigenous clients in our Community Programs.
Key Responsibilities- Provide clinical services to Indigenous clients, including assessment, diagnosis, and treatment planning.
- Work collaboratively with multidisciplinary teams to develop and implement care plans.
- Conduct home visits and assessments to determine client needs and develop supportive and rehabilitative programs.
- Facilitate client access to community services and resources, including referrals to other healthcare professionals.
- Provide education and support to clients, families, and staff on mental health and substance use issues.
- Participate in quality improvement and risk management activities to ensure the delivery of high-quality care.
- Collaborate with community outpatient clinics and treatment programs to ensure seamless care transitions.
- Bachelor's Degree in Social Work, Clinical Counselling, or a related field.
- Current full registration with the BC College of Social Workers or a related professional organization.
- Two years of relevant clinical experience in a psychiatric acute, community, or outreach setting, including one year of experience working with Indigenous clients.
- Valid B.C. driver's license and access to a personal vehicle for business-related purposes.
- Demonstrated knowledge and understanding of Indigenous cultural practices and beliefs related to mental health and substance use.
- Experience working with Indigenous populations and communities.
- Knowledge of community resources and methods of accessing them, particularly in Indigenous communities.
