Salary Range: $65,000 - $80,000 per year (dependent on experience)

Job Summary:

The Community Palliative Care Nurse Specialist will be responsible for providing expert nursing care to individuals and families in their homes and communities. The successful candidate will have a strong background in palliative care and excellent communication skills, with the ability to work collaboratively as part of a multidisciplinary team.

Key Responsibilities:

  • Provide comprehensive nursing assessments and develop individualized care plans
  • Collaborate with healthcare professionals to ensure seamless transitions of care
  • Develop and implement education programs to empower clients and families
  • Maintain accurate records and reports

Required Skills and Qualifications:

  • Licensed Registered or Licensed Practical Nurse in good standing
  • Minimum 3 years' experience in palliative care
  • Strong communication and interpersonal skills
  • Able to work independently and collaboratively as part of a team
