Occupational Therapist II

We are seeking a highly skilled and compassionate Occupational Therapist II to join our Primary Care team at Northern Health Authority. As an Occupational Therapist, you will work collaboratively with interprofessional teams to develop and implement plans of care that promote optimal health and engagement in daily activities for our patients.

• Conduct comprehensive assessments and develop treatment plans that address the unique needs of each patient
• Provide evidence-based interventions and education to patients and their families to promote independence and participation in daily activities
• Collaborate with healthcare teams to ensure seamless care coordination and communication
• Participate in quality improvement initiatives to enhance patient outcomes and experiences

• Graduate of a recognized Occupational Therapy program
