Job DescriptionIn this role, you will be responsible for delivering high-quality patient care, assessing individualized needs, and executing care plans. You will work collaboratively with patients to build trust and rapport, engaging them in a collaborative approach to achieve optimal outcomes.
You will also maintain accurate professional patient records related to patient encounters and plans, ensuring seamless communication among healthcare professionals.
Key Responsibilities- Assess patients' individualized needs using appropriate assessment tools and techniques
- Execute and continuously monitor care plans to ensure optimal patient outcomes
- Engage patients using a collaborative approach, building rapport and trust
- Maintain and update professional patient records related to patient encounters and plans
To be successful in this role, you will need:
- Registration with the CNO as a Registered Nurse in good standing
- A valid driver's license and access to a vehicle is required
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
- Ability to work independently with minimal supervision
