Key Responsibilities:
  • Conduct comprehensive biopsychosocial assessments to identify patients' emotional, physical, cognitive, and social needs, risks, and strengths.
  • Develop and implement individualized care plans in collaboration with the multidisciplinary team and community partners.
  • Provide guidance and support to patients and families in developing coping skills and crisis plans.
  • Collaborate with healthcare professionals to ensure seamless care transitions and effective discharge planning.
  • Advocate for patients' rights and interests, and provide consultative social work services to the treatment team and hospital staff.
Requirements:
  • Master's Degree in Social Work from an accredited program.
  • Registration with the Ontario College of Social Workers & Social Service Workers (OCSWSSW).
  • Minimum of one year of experience as a staff Social Worker in an acute care setting.
  • Geriatric experience is an asset.
  • Excellent clinical skills, interpersonal skills, and ability to work effectively in a fast-paced environment.
