We are seeking a skilled Registered Nurse to join our Home Living team in the Edmonton Zone Continuing Care. This position offers an exciting opportunity to provide professional support services in the homes of clients of all ages and stages of life.
Job Summary:
This role is responsible for providing a wide range of nursing services to patients, families, communities, and populations while ensuring their safety and well-being. As a self-directed team member, you will work collaboratively with the healthcare team and have responsibilities including assessment of needs, delivering appropriate therapeutic nursing services within professional practice standards, coordinating resources, establishing goals for client delivery, and evaluating outcomes.
Key Responsibilities:
- Provide safe, quality patient and family-centered care while reflecting the shared vision and values of Alberta Health Services.
- Assess the needs of clients and develop individualized care plans to meet their unique requirements.
- Collaborate with the healthcare team to coordinate resources and ensure seamless care transitions.
- Evaluate the effectiveness of care plans and make adjustments as needed.
Requirements:
To be successful in this role, you must have:
- A completion of an accredited nursing education program.
- An active or eligible registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- A current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
- Two years of recent nursing experience or completion of a refresher program within 2 years.
- The ability to work independently and use sound professional judgment.
- Excellent organizational skills, communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment.
Preferred Qualifications:
Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Palliative care experience, previous Continuing Care and/or geriatric experience, and experience with Meditech, Connect Care and the RAI Home Care assessment tool are also desirable.
