Job Summary

We are seeking a highly skilled and compassionate Case Management Specialist to join our team at Accoravillage. As a Case Management Specialist, you will work directly with new immigrants and refugees, providing them with the support and resources they need to settle into life in Canada.

Key Responsibilities
  • Assess client needs and develop personalized service plans to address their unique challenges and goals.
  • Provide case management services to ensure that clients' needs are met, including connecting them with community resources and advocating on their behalf.
  • Conduct individual, family, and group orientation sessions on relevant settlement topics, such as living and working in Canada.
  • Establish networks with other settlement professionals, community groups, and cultural organizations to build on the strengths of newly arrived families.
  • Collect client feedback to track outcomes of settlement services and make data-driven decisions to improve our programs.
  • Keep accurate and up-to-date client files, ensuring confidentiality and adhering to all relevant regulations.
  • Prepare and submit required reports and statistics to stakeholders.
  • Represent Accoravillage in a professional manner in various community groups and committees, promoting positive public relations and advancing our mission.
Requirements
  • A degree in Social Work or a related field, or equivalent experience.
  • Fluency in Somali and additional language skills, such as Arabic or French, are an asset.
  • Minimum 2 years' experience working with newcomers to Canada, with a strong understanding of their unique challenges and needs.
  • Certification as a Settlement Practitioner through a recognized organization is desired.
  • A valid driver's license and a reliable vehicle for transporting clients, as well as a clean driving record.
  • A Police Information Check and Child Intervention Record Check, both current within the past six months.
