Your Key Responsibilities:

  • Using appropriate assessment tools and techniques to evaluate patients' individualized needs
  • Executing and continuously monitoring care plans tailored to each patient's unique requirements
  • Engaging patients through a collaborative approach by building rapport and trust
  • Maintaining and updating professional patient records related to patient encounters and plans

Your Qualifications:

  • Registration with the CNO as a Registered Nurse in good standing
  • A valid driver's license and access to a vehicle are required
  • Experience in delivering community care is an asset, but not essential
  • Reliability, flexibility, and a willingness to travel to and from patient residences within a designated area
  • A genuine passion for making a positive impact in your community
  • Required on-call coverage, including after-hours to ensure patient safety and meet patient care needs
