Job SummaryWe are seeking a skilled and compassionate Health Care Social Worker to join our team at Nova Scotia Health. As a Health Care Social Worker, you will play a critical role in providing psychosocial assessments, adjustment therapy, supportive therapy, crisis counselling, resource counselling, and discharge planning and case management to our patients and their families.
Key Responsibilities- Provide psychosocial assessments to identify patients' needs and develop individualized care plans
- Deliver adjustment therapy, supportive therapy, crisis counselling, and resource counselling to patients and their families
- Develop and implement discharge plans and provide case management to ensure a smooth transition for patients
- Collaborate with interdisciplinary teams to provide comprehensive care to patients
- Provide consultation and education to patients, families, and healthcare teams on social and emotional issues
- Master's Degree in Social Work from a recognized university
- Registered or eligible for registration with the NSCSW
- Two (2) years of clinical experience as a clinical Social Worker preferred
- BCLS
- Strong counselling and communication skills
- Capacity to achieve a high degree of autonomous practice
- Ability to work within a multidisciplinary team context
- Knowledge of issues related to family violence, sexual abuse, addictions, grief and loss, end of life, disability, crisis intervention, caregiving and continuum of care and intervention skills that come from knowledge of theoretical frameworks/approaches to practice
- Competencies in other languages an asset, French preferred
- Competitive hourly rate of $49.05
