About the Job
This exciting opportunity is available for a short assignment, full-time/75 hours bi-weekly position, with shifts including days, evenings, weekends, and holidays. You will be working in a fast-paced, dynamic environment, collaborating with interprofessional teams to provide exceptional care to our clients. As a Continuing Care Coordinator, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities
- Assess clients' physical, emotional, and social needs and develop personalized care plans
- Collaborate with interprofessional teams to facilitate hospital discharge and ensure seamless transitions to community-based care
- Authorize services and make referrals/linkages to other professional and volunteer resources as needed
- Maintain ongoing communication with clients, family members, caregivers, and healthcare providers to ensure coordinated care
Requirements
- Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
- Current and ongoing registration (or eligibility for registration) with the applicable Nova Scotia Professional Regulatory body
- Minimum three years' experience with the professional regulatory body
- Demonstrated knowledge and/or experience in a community or long-term care setting
- Knowledge and/or experience in dedicated case management or discharge planning
- Competent with computer use, keyboarding, and able to conduct assessments using a laptop
- Knowledge of community agencies and resources
