About the Job

This exciting opportunity is available for a short assignment, full-time/75 hours bi-weekly position, with shifts including days, evenings, weekends, and holidays. You will be working in a fast-paced, dynamic environment, collaborating with interprofessional teams to provide exceptional care to our clients. As a Continuing Care Coordinator, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities

  • Assess clients' physical, emotional, and social needs and develop personalized care plans
  • Collaborate with interprofessional teams to facilitate hospital discharge and ensure seamless transitions to community-based care
  • Authorize services and make referrals/linkages to other professional and volunteer resources as needed
  • Maintain ongoing communication with clients, family members, caregivers, and healthcare providers to ensure coordinated care

Requirements

  • Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current and ongoing registration (or eligibility for registration) with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience with the professional regulatory body
  • Demonstrated knowledge and/or experience in a community or long-term care setting
  • Knowledge and/or experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
