About the Role

Vancouver Coastal Health is seeking a skilled Registered Nurse to join our Transition Services team in Vancouver. As a Transition Services Coordinator, you will play a critical role in ensuring seamless transitions between acute and community care settings.

Key Responsibilities

  • Lead and facilitate acute to community transitions by assessing and evaluating clinical and functional needs, and coordinating community staff and resources to ensure effective care planning and delivery.
  • Develop and implement policies, procedures, and standards for the transition services program in community care, ensuring alignment with VCH's strategic objectives.
  • Coach and encourage acute and community colleagues to adopt new and evolving clinical pathways and practices, promoting a culture of excellence and innovation.
  • Collaborate with care teams and other healthcare professionals to plan and facilitate safe client discharges, providing expert advice on community services and resources.
  • Make informed community referrals based on client needs, urgency, and best practices, ensuring timely access to appropriate treatment and support.
  • Coordinate the transition of clients to long-term care, utilizing clinical decision support tools to inform care planning and delivery.
  • Maintain accurate and up-to-date client records and clinical assessments, ensuring compliance with VCH's quality and safety standards.
  • Participate in committees and working groups as directed, contributing to the development of policies, procedures, and standards that support the transition services program.

Qualifications

To be successful in this role, you will require:

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Five (5) years' recent, related experience in a community health setting, with a minimum of three (3) years in a leadership or coordination role.
  • Proven expertise in quality improvement, research, and evaluation activities, with a focus on clinical decision support and severity assessment.
  • Strong knowledge of nursing theory and practice, with a focus on client and family-centered care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including clients, families, and healthcare professionals.
