We are seeking a skilled Social Work Liaison to join our team at Fraser Health. As a key member of our healthcare team, you will play a vital role in facilitating the discharge planning process for patients transitioning from acute care to community-based services.
Key Responsibilities- Assess client care needs and coordinate referrals to community services, including Home Health, Home Support, and Residential care.
- Collaborate with interdisciplinary teams to develop client care plans and ensure timely discharge from acute care facilities.
- Liaise with clients, families, physicians, and other healthcare professionals to arrange for community services and resources.
- Facilitate the exchange of information between acute care facilities and community services to ensure seamless care transitions.
- Participate in quality improvement activities and contribute to the development of indicators, standards, and practices.
- Bachelor's degree in Social Work from an approved school.
- Two years' recent related clinical experience working with complex geriatric patients and adults with chronic or acute illnesses.
- Current full registration with the BC College of Social Workers.
As a Social Work Liaison with Fraser Health, you will have the opportunity to work with a dynamic team of healthcare professionals and make a meaningful difference in the lives of our patients and their families.
