Job Summary:

At Brightshores, we are seeking a highly skilled and compassionate Patient Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

Key Responsibilities:

  • Assist with routine activities of patient care under the guidance of an RN and RPN, ensuring a high level of patient satisfaction and well-being.
  • Collaborate with healthcare professionals to promote a safe and supportive environment for patients.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in ongoing education and training to enhance knowledge and skills in patient care and support.
  • Contribute to a culture of patient safety and quality care, adhering to Brightshores' 'CARE' Behaviours and 'LEAD' Competencies.

Requirements:

  • Certificate of Completion of the Personal Support Worker Program from a recognized Community College or equivalent experience and education.
  • Current CPR certification.
  • Recent experience in a healthcare facility, preferably in an acute care setting.
  • Excellent written and verbal communication skills, with the ability to work collaboratively with diverse teams.
  • Working knowledge of Infection Control Practices and a commitment to maintaining a safe and healthy work environment.
  • Ability to work all shifts, including evenings, weekends, and holidays, as required.
  • Recent satisfactory performance and attendance record, with a commitment to ongoing professional development.
