Client Care Coordinator

Spectrum Health Care is seeking a highly skilled and compassionate Client Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the highest level of care and support for our clients.
Key Responsibilities:- Client Support: Collaborate with community PSWs and Nurses to schedule and coordinate services, ensuring seamless care delivery.
- Liaison and Communication: Build strong relationships with clients, staff, and community partners, facilitating effective communication and problem-solving.
- Administrative Tasks: Collect and enter client data into our computer system, providing accurate and timely information to support our healthcare services.
- Program Support: Assist with scheduling, lists, and other administrative duties to support our PSW and Nurse programs.
- Experience: Minimum of 2 years working in an office setting, with a strong background in customer service and administrative tasks.
- Skills: Excellent English written and verbal communication skills, with the ability to multitask and prioritize tasks effectively.
- Education: High school diploma or equivalent, with a strong foundation in medical terminology and computer skills.
