Job Summary


We are seeking a skilled Licensed Practical Nurse to join our team at Fraser Health. As a key member of our Home Health Services team, you will play a vital role in providing high-quality care to our clients.



Key Responsibilities

  • Conduct initial client consultations, assessments, and interventions to determine client needs and develop care plans
  • Screen referrals and confirm client eligibility for services
  • Prepare and maintain accurate client records and provide intake/referral data and statistics
  • Participate in team meetings, continuing education, and engage in ongoing team planning and quality improvement
  • Review, prioritize, and process requests for home-based, ambulatory, or other community-based services
  • Coordinate the flow of care by liaising with healthcare professionals and organizations
  • Provide education and counseling to clients, family members, and caregivers
  • Communicate and consult with the healthcare team to ensure continuity of care
  • Maintain and complete required documentation


Qualifications

  • Graduation from an approved program of Practical Nursing, plus one year of recent related acute, residential, and/or community-based experience
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
  • Certificates in CPR Level "C" or equivalent and First Aid
