Primary Care Nurse

We are seeking a motivated Registered Nurse with a dedication for delivering compassionate patient care to join our team in Primary Care.
Job SummaryThe ideal candidate will have three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings, including experience developing and providing formal education to patients, families, the public, or an equivalent combination of training and experience.
Key Responsibilities:- Provide direct clinical care and services to individuals, groups, and communities in settings such as physician practices, homes, schools, and communities.
- Develop and implement care plans that meet the unique needs of each patient, incorporating their personal goals and preferences.
- Collaborate with interprofessional teams to ensure seamless delivery of care and promote continuity across settings.
- Foster strong relationships with patients, families, and community members to build trust and ensure optimal health outcomes.
