Key Responsibilities

As a Nursing Professional with CarePartners, your primary responsibilities will include:

  • Developing and maintaining effective Care Plans in collaboration with community health services and Ontario Health atHome.
  • Providing exceptional nursing services to patients both in-person and virtually.
  • Communicating effectively with patients, family members, and interdisciplinary care teams.
  • Traveling regularly to visit patients in their homes, retirement homes, or other settings as required.

Requirements

To succeed in this role, you must possess:

  • Valid driver's license and vehicle for regular travel.
  • Excellent English verbal and written communication skills.
  • Current immunization record, including a two-step TB test.

CarePartners serves the community through clinics, transitional care units, and relief services in retirement homes and shared care settings. We also offer outreach medical care and clinics in countries with limited access to healthcare through our Community Nursing Services program.
