Case Manager

Job Summary:

The Case Coordinator provides comprehensive assessments to clients, ensuring a high-quality client experience. This role involves developing care plans in consultation with clients and caregivers, managing care budgets, and navigating clients to appropriate resources.

Key Responsibilities:

• Conduct comprehensive assessments in clients' homes using conversation-based methodology.
• Develop and conduct therapeutic rapport with clients.
• Utilize technology to complete assessments and develop care plans.
• Identify and prioritize environmental risks for clients' health and safety.
• Facilitate access to client-identified services to support care plans and goals.
• Make referrals to community and health resources as required.
• Monitor client progress and adjust care plans as needed.
• Review client care plan expenditures and support clients to manage within budgets.

Education, Designations and Experience:

• University degree or college diploma in the nursing field.
• Minimum of 3 years related experience in a home and community care organization, including experience in case management.
• Broad working knowledge of community resources, including services and programs within defined catchment areas.
• Proven experience in building external and community relationships.
