Job Summary:

As a member of our team, you will have the opportunity to work in a diverse and dynamic environment, providing physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, and counseling to individuals and families.

Key Responsibilities:
  • Provide and manage high-quality nursing care: Develop and implement individualized care plans, conduct thorough assessments, and provide evidence-based interventions to promote optimal health outcomes.
  • Teach and delegate tasks: Educate and empower clients and their families to manage their health, delegate tasks and functions to different levels of workers, and provide guidance on self-care and disease management.
  • Collaborate with the healthcare team: Participate in interdisciplinary meetings, contribute to care planning, and work collaboratively with other healthcare professionals to ensure comprehensive care.
  • Meet regulatory requirements: Comply with all relevant laws, regulations, and standards, including those related to patient confidentiality, consent, and documentation.
Requirements:
  • Current registration: Be currently registered with the College of Nurses of Ontario.
  • CPR and First Aid certification: Hold current CPR and First Aid certification.
  • Experience and skills: Possess experience working with pediatric patients with chronic conditions, demonstrated skills in observation, critical thinking, independent assessment, health teaching, communication, and interpersonal relationship building.
  • Education and training: Have a strong foundation in nursing principles, practices, and research, and be committed to ongoing learning and professional development.
