Job Summary

We are seeking a skilled and compassionate Nurse Care Specialist to join our team at RT Medical. As a Nurse Care Specialist, you will provide high-quality patient care and support to individuals in their homes, clinics, and other settings.

Key Responsibilities
  • Assess and Plan Care: Conduct thorough assessments of patients' needs and develop personalized care plans to meet their unique requirements.
  • Provide Skilled Nursing Care: Deliver high-quality nursing care to patients, including wound care, IV medications, and other clinical duties as required.
  • Collaborate with Healthcare Teams: Work closely with patients, families, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Travel and Flexibility: Travel within your region to provide care to patients in their homes, clinics, and other settings, and be flexible to meet the changing needs of our patients.
  • Documentation and Reporting: Maintain accurate and detailed records of patient care, including progress notes, medication administration records, and other relevant documentation.
Requirements
  • Education and Certification: Bachelor of Science in Nursing degree (BScN) or equivalent, and current registration with the College of Nurses of Ontario.
  • Experience: At least 1 year of experience as a Registered Nurse or Registered Practical Nurse, preferably in a home health or community care setting.
  • Skills and Abilities: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and strong problem-solving and time management skills.
  • Physical Demands: Ability to stand, bend, walk, crouch, kneel, and reach, and to lift 50lbs (23 kg) independently.
