We are seeking a highly skilled Clinical Pharmacist II to join our Primary Care team at Northern Health Authority. As a key member of our healthcare team, you will provide clinical pharmacy services to patients, working closely with healthcare professionals to identify and resolve actual and potential drug therapy problems.
Key Responsibilities- Provide comprehensive medication management to patients, including assessment, treatment, and education.
- Collaborate with healthcare professionals to develop and implement patient care plans.
- Conduct medication reviews and provide recommendations for optimization of medication regimens.
- Participate in interprofessional teams to provide patient-centered care.
- Develop and maintain knowledge of medications, including their indications, contraindications, and potential interactions.
- Current registration with the College of Pharmacists of British Columbia.
- Minimum 2 years recent, related patient care experience, ideally in a team-based primary care setting or equivalent.
- A valid Class 5 driver's license and access to a personal vehicle for business-related purposes.
- Comprehensive benefit packages, including extended health/dental and a municipal pension plan.
- Four weeks vacation with one year of continuous service.
