About the Role

We are seeking a highly skilled Mental Health Professional to join our team at Fraser Health, providing treatment and care management services to individuals with mental illness and concurrent disorders.

As a key member of our interprofessional care team, you will work collaboratively to deliver person-centered and recovery-oriented care. Your primary focus will be on coordinating care for clients, conducting mental health assessments, developing comprehensive treatment plans, and administering prescribed medications.

Main Responsibilities
  1. Care Coordination: Collaborate with the interprofessional care team to coordinate client care, perform intake functions, conduct mental health assessments, and develop, implement, and maintain comprehensive treatment plans based on client needs and best practices.
  2. Nursing Care: Plan and provide direct nursing care to clients in compliance with standards of practice, performing activities such as reviewing client/family history, conducting physical/psychosocial assessments, assessing vital signs, and collaborating with other members of the healthcare team to discuss and review the care plan.
  3. Therapeutic Relationships: Establish, maintain, and enhance therapeutic relationships with clients and their informal support network by engaging stakeholders in treatment planning and evaluation, providing support and follow-up planning, promoting self-care, encouraging informed decision-making, and empowering clients to improve their quality of life.
  4. Treatment Services: Provide treatment services, including crisis intervention, individual counseling, and education, according to various theoretical models, to clients and their informal support network as required.
  5. Collaboration and Communication: Consult and collaborate with members of the interprofessional care team, including MH&SUS Physicians, Primary Care Providers, Team Coordinator, Clinical Nurse Educators, and other healthcare professionals/providers, in the identification and resolution of issues, adjustments to care plans as required, and in regard to changes in client condition.
Requirements
  • Education and Experience: Graduation from an approved School of Nursing or Psychiatric Nursing, with two years of recent related experience providing treatment services to individuals with mental health and/or concurrent disorders.
  • Skills and Abilities: Ability to complete initial and ongoing mental health and substance use client assessment and provide nursing care through appropriate/prescribed technical, therapeutic, safety-type interventions, including crisis intervention; counselling and group facilitation skills; knowledge of legislation and government policies and procedures that relate to mental health and substance use practices; ability to communicate effectively verbally and in writing with clients, families/caregivers, the public, medical staff, and members of the interprofessional care team; and ability to promote person-centered care that demonstrates empathy for clients/families/caregivers, sensitivity to diverse cultures and preferences, including advocacy for special populations.
Compensation

The salary range for this position is approximately CAD $90,000 per year, depending on qualifications and experience. This is a challenging yet rewarding role that offers opportunities for professional growth and development within a dynamic and supportive team environment.
