Job Summary
The Physician Navigator plays a vital role in supporting the vision and mission of Prairie Mountain Health by reducing administrative burdens on physicians and providing practice-specific support. This ambassador will interact with medical providers and allied health workers to improve navigation of available resources for patients and practices.
Key Responsibilities
- Develop and maintain physician orientation processes to enhance knowledge of regional and provincial services.
- Conduct regular facility tours to gather feedback from new physicians on additional information needed during orientation.
- Provide timely support through phone calls, in-person requests, and online services.
- Curate and maintain a directory of services in PMH in collaboration with clinics, hospitals, managers, and physician groups.
- Work with Diagnostic Services to understand available tests, their locations, and ordering procedures.
- Ensure seamless computer access for new physicians.
- Offer a comprehensive overview of PMH services tailored to specific communities.
- Arrange dedicated orientation time for new physicians with Department Chiefs, Site CMOs, or Clinic physicians.
- Collaborate with communities receiving new physicians to curate local resources, including family support information.
- Regularly check-in with new physicians to the region.
- Tour facilities and clinics to gather feedback from established physicians on patient navigation issues.
- Perform exit interviews and report to Executive Management on notable issues and patterns.
- Define available procedures in PMH.
- Outline referral processes for PMH and Winnipeg.
- Identify clinical issues that can be addressed virtually.
- Determine services requiring physician referrals versus those accessible without.
- Document community resources and access methods.
- Communicate new or updated services to physicians.
- Perform other duties as assigned.
