Job Summary

Provide compassionate care and support to patients and families affected by substance use issues.

Vancouver Coastal Health is seeking a skilled Substance Use Specialist to join our team at Lions Gate Hospital in North Vancouver, BC.

As a Substance Use Specialist, you will work collaboratively with healthcare professionals and community agencies to provide patient-centered care and support.

Key Responsibilities:

  • Provide direct support to patients and families through assessment, referrals, and system navigation.
  • Collaborate with the interdisciplinary team to develop and implement patient care plans.
  • Provide education and support to patients and families on substance use issues and related health concerns.
  • Develop and maintain relationships with community agencies and healthcare providers to ensure seamless patient care.

Requirements:

  • Master's degree in Social Work or related field.
  • Current registration with the British Columbia College of Social Workers.
  • Minimum 2 years of experience in a related field.
