Job Overview

We are currently seeking a Transitional Care Assistant (CCA) to join our team at the West Bedford Transitional Health Centre. As a valued member of our staff, you will contribute to delivering high-quality patient care and support.

Salary and Benefits

The estimated annual salary for this position is $39,200 - $47,240, based on a full-time schedule. Comprehensive benefits, including health and dental plans, RRSP program, vacation accrual, travel insurance, access to virtual healthcare, free onsite parking, and WorkPerks discounts, will be provided.

Job Description
  • Assist patients with daily living activities, such as bathing, dressing, oral care, skin care, meals, and mobilizing, according to their care plan.
  • Collaborate with the interdisciplinary team in developing individualized patient care plans.
  • Plan and implement patient leisure activities on a daily basis.
  • Perform light housekeeping duties.
  • Communicate and report relevant information regarding patient care or safety to the Supervisor.
Requirements

To be successful in this role, you will need:

  • A valid Continuing Care Certification or Provincial Personal Care Worker course or equivalent.
  • Registration in Nova Scotia's CCA registry as per the Continuing Care Assistants Registry Act.
  • Flexibility to work a variety of shifts, including days, nights, weekends, and holidays.
  • A clear criminal record with vulnerable sector screening.
