Job Summary

We are seeking a highly skilled Registered Practical Nurse to join our Assertive Community Treatment Team. The successful candidate will work collaboratively with a multidisciplinary team to provide comprehensive care to individuals with serious and persistent mental illness.

Key Responsibilities
  • Conduct mental and physical health assessments, develop plans, and provide treatment and rehabilitation services
  • Work collaboratively with the ACT team to manage and administer medication within scope of practice
  • Provide follow-up nursing care and monitoring to clients referred to the Clozapine Clinic
  • Conduct mental and physical health assessments, make referrals to community resources, and provide support and education to clients and their families
Requirements
  • Licensed Registered Practical Nurse (Dipl. N.)
  • Member in good standing with the Ontario College of Nurses
  • Four or more years' experience as an RPN, preferably in a mental health setting
  • Excellent health assessment skills, sound clinical judgment, and diagnostic skills
  • A second language (French, Cantonese, Mandarin, Farsi, Italian, Korean, Punjabi, Russian, Tamil, Urdu) reflecting the local community is preferred
Working Conditions

Predominantly office setting, community settings, meetings at client homes, and travel with clients.
