Nurse Case Manager

About the Role
Vancouver Coastal Health is seeking a skilled Registered Nurse or Registered Psychiatric Nurse to join our Long Term Care team in Sechelt, BC.
Key Responsibilities
- Manage a caseload of clients with chronic illnesses or long-term physical disabilities, providing comprehensive care and support.
- Conduct assessments, identify problems, and develop plans of care in collaboration with interdisciplinary teams and community agencies.
- Provide continuity of care across the continuum, coordinating with healthcare professionals and community resources to ensure seamless transitions.
- Develop and maintain partnerships with General Practitioners and other healthcare providers to ensure comprehensive care.
- Act as a clinical resource for the interdisciplinary team, providing education and guidance on chronic disease management and self-care principles.
- Consult and collaborate with multidisciplinary teams, healthcare professionals, and community agencies to achieve excellence in client-centered care.

Qualifications
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Three years' recent clinical nursing experience, preferably in acute care or case management.
- Valid BC Driver's License and ability to travel locally.
