Estimated Salary: $47.38 - $57.71 per hourWe are seeking a highly skilled Nurse Care Specialist to join our team. As a key member of our care delivery team, you will play a critical role in ensuring the health, safety, and well-being of our clients.
Job Description:This is an exciting opportunity for a compassionate and detail-oriented nurse to make a meaningful difference in the lives of our clients.
- Provide high-quality nursing care to clients in their homes, community settings, or residential facilities;
- Conduct thorough assessments, develop individualized care plans, and implement interventions to address client needs;
- Collaborate with interdisciplinary teams, including physicians, therapists, social workers, and other healthcare professionals;
- Communicate effectively with clients, families, and caregivers to ensure seamless care transitions;
- Monitor client progress, adjust care plans as needed, and document all interactions and interventions.
To be successful in this role, you must possess:
- A current registration with the BCCNM or EMALB as an RN or LPN;
- Previous experience in a healthcare setting, preferably in a community-based or long-term care environment;
- Strong assessment and communication skills, with the ability to work independently and as part of a team;
- A commitment to delivering person-centered care that respects the dignity and autonomy of each client;
- A valid Class 5 driver's license (Class 4 preferred).
