About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Mental Health Direct Care Nurse. As a key member of our Home and Community Care Support Services team, you will play a vital role in providing high-quality care and support to individuals in their homes.

Key Responsibilities
  • Assess mental health status and provide ongoing risk assessment
  • Monitor medication management and liaise with attending physicians, psychiatrists, and other support services
  • Provide education and supportive counseling to patients, family, and caregivers
  • Assess physical health and provide clinical nursing care where appropriate
  • Provide education regarding mental health, medication, and coping strategies to patients, family, and caregivers
Requirements
  • Membership, in good standing, with the College of Nurses of Ontario - Registered Nurse (BScN or diploma)
  • Three – five years of relevant recent experience as a Registered Nurse in Mental Health
  • CNA Mental Health Certificate, or a comprehensive Mental Health Education Program that has been reviewed and approved by Ontario Health atHome in writing
  • Certification in mental health nursing
  • Proficient in the operation of a personal computer in a networked environment using a variety of computer software and data entry skills
  • Knowledgeable in the use of technology in a community setting
  • Demonstrated skill in setting and meeting deadlines
  • Demonstrated effective use of time and tools to manage work efficiently
  • Demonstrated experience in comprehensive assessment, use of different or combined approaches
  • Demonstrated skills in synthesizing information in order to plan and adjust services and to assist clients with informed decision-making
  • Demonstrated skill in crisis management and risk situations
  • Requires analysis and critical thinking skills
  • Working knowledge of community resources and roles of health care professionals
  • Solid knowledge of health care related legislation and practices
  • Knowledge of direct care/case management models used in community health care organizations
  • Knowledge of Ontario Health atHome priorities, policies, practices, and service standards
  • Effective interpersonal and communication skills
  • Effective organizational and planning skills
  • Basic proficiency with computerized information systems
  • Effectively maintain a constant flow of verbal and written communication with others throughout the workplace as well as outside the organization
  • Able to communicate with clients', their families, and other relevant individuals in order to follow through with care plan directives
  • Demonstrated awareness of cultural diversity, as well as ability to behave discreetly and sensitively to confidential issues
Preferred Qualifications
  • Experience working with diverse patient groups
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
