Mental Health Direct Care Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Mental Health Direct Care Nurse. As a key member of our Home and Community Care Support Services team, you will play a vital role in providing high-quality care to individuals in their homes.
Key Responsibilities- Assess the mental health needs of clients and develop personalized care plans to ensure effective support and linkage with community resources.
- Provide timely and effective mental health nursing interventions in the home, including medication reconciliation and education.
- Collaborate with healthcare professionals, community agencies, and clients to ensure seamless care coordination and communication.
- Conduct comprehensive assessments, identify risks, and develop strategies to mitigate them.
- Provide education and support to clients, families, and caregivers on mental health, medication management, and coping strategies.
- Participate in case management and care planning to ensure clients receive the necessary support and services.
- Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse (BScN or diploma).
- Three to five years of recent experience as a Registered Nurse in mental health.
- CNA Mental Health Certificate or a comprehensive mental health education program approved by Ontario Health atHome.
- Certification in mental health nursing.
- Proficiency in computer software and data entry skills, including Excel and database software.
- Knowledge of community resources, health care legislation, and practices.
- Effective communication, interpersonal, and organizational skills.
- Able to maintain confidentiality and handle sensitive information.
- Experience working with diverse patient populations, including multicultural, homeless, palliative, and acquired brain injury (ABI) clients.
- Case management experience or recent community experience.
- Ability to speak French or another second language.
