Job Title: Rehabilitation Assistant - Ontario Home Care Therapy Services

Job Summary:

The Rehabilitation Assistant will work under the direction of a supervisor to ensure the delivery of a treatment program as prescribed by a regulated health care practitioner. The successful candidate will support adult clients to help them achieve their maximum potential.

Key Responsibilities:

  • Carry out the prescribed treatment program as directed and taught
  • Document the clients' progress and report outcomes to the applicable regulated health care provider
  • Implement rehabilitation programs with clients as directed by Physiotherapists or Occupational Therapists
  • Promote maximum independence of clients in the home to support re-integration into the community
  • Implement exercise programs or group exercise classes as directed
  • Implement behavioral interventions with clients as directed
  • Facilitate and supervise activities of daily living, along with social, educational/vocational re-integration plans, as directed
  • Facilitate and implement cognitive therapy programs as directed
  • Promote and implement leisure activities as directed
  • Attend case conferences and team meetings as required
  • Understand and implement infection prevention practices
  • Participate in ongoing internal and/or external continuing education activities
  • Adhere to Bayshore Policies and Procedures
  • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System
  • Participate in proactive Health & Safety activities while performing all duties
  • Maintain confidentiality of client and corporate information and discuss same only with appropriate Bayshore personnel

Qualifications:

Education: Completion of a recognized Physiotherapy, Occupational Therapy, and/or Rehabilitation Assistant College Program

Skills and Abilities:

  • Enjoy dealing with people and working in a fast-paced customer service environment
  • Strong communication (oral and written) and presentation skills
  • Exceptional interpersonal skills. Ability to handle difficult situations in an objective consistent manner
  • Ability to work independently and effectively collaborate with a team
  • Ability to be self-directed and take direction
  • Demonstrated knowledge of personal computers and proficiency with secure email. Commitment to continual learning
  • Valid driver's license and auto insurance
