Key Responsibilities:- Provide comprehensive health assessments and develop individualized care plans for patients with complex mental health and addiction needs.
- Collaborate with consulting physicians and interdisciplinary teams to ensure seamless care coordination and effective treatment outcomes.
- Utilize evidence-based practices and established care paths to guide care decisions and optimize patient outcomes.
- Support patients and families through comprehensive initial assessments, individualized treatment plans, and ongoing care coordination.
- Document patient information accurately and communicate effectively with healthcare teams to ensure continuity of care.
- 3+ years of experience as a Nurse Practitioner, with a focus on mental health and addiction expertise.
- Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario.
- Active membership in related professional associations, such as NPAO and RNAO.
- Mental Health Nursing certificate preferred.
- Masters of Nursing required.
- Innovative and professional role model with excellent assessment and clinical skills.
- Ability to prioritize and organize multiple demands, work autonomously, and possess a working knowledge of the Mental Health Act and community mental health resources.
