Job Description

As a Community Nurse with CarePartners, your primary responsibilities will include:

  • Developing and maintaining effective care plans for patients receiving community health services.
  • Providing exceptional nursing services to patients in their homes, clinics, and other settings.
  • Communicating and collaborating with patients, family members, and other healthcare professionals to ensure seamless care.
Requirements

To be successful as a Community Nurse with CarePartners, you will need:

  • A valid RN or RPN degree or diploma.
  • Current College of Nurses of Ontario (CNO) registration in good standing.
  • Exceptional interpersonal and communication skills.
  • A valid driver's license and vehicle for regular travel.
